We don't do TM work, and we are 99.9% single family residences/apartments/condo's. But to answer the original question we charge $15 extra if the front door is on the 2nd floor. $50 if the front door is on the 3rd floor. Both of those are if there is no elevator. Hardly ever do anything on the 3rd floor, very few places like that around here.
